How to Apply for a Utah Educator License

Request a Copy of Your Transcripts

You must turn in an official USU transcript showing your degree posted. Please go to the National Student Clearinghouse (link below) to request a transcript. If ordering before your degree has posted, make sure you check the box that says “hold until degree is awarded”. In order to be considered official, a paper transcript must be in a sealed envelope. Official electronic transcripts must come directly to Lisa Christensen at licensing@aggies.usu.edu from either the university or the clearing house. Choose the option to have it sent to "employer or other" and enter Lisa Christensen's name in the recipient box and licensing@aggies.usu.edu in the email box.

If you earned any credits at another institution that were applied to your USU degree, you must order an official transcript from that institution. An official USU transcript DOES NOT print classes from other universities.

If not Current, Complete the Utah Educator Ethics Review

You must have completed the Ethics Review within one year of licensure, so it's possible that the Ethics Review that you completed in order to apply for your program will need to be renewed. Complete or renew your Ethics Review in the USIMS system: usims.schools.utah.gov

If not Current, Complete a Background Check

You will need to have a background check completed prior to your application for your Educator License. Background checks are valid for a period of five years before licensure.

If you hold a current teaching license, you will not have to complete another background check. We cannot accept copies of background checks done for a school district or other employment. To apply for a background check, go to USIMS and sign in or sign up: usims.schools.utah.gov

Complete the Program Survey

File your Application and Attach your Application Materials

Log in to the eGrad application portal using your A-number and Strong Password using the link below. Once logged in, click on "Submit New Form", go to the "Licensing" subcategory, and click on "Licensure Application". Fill out the application, upload all of your documents, and click on "Submit".
