Applications for Logan and Price Campuses
Applications for the 2026-2027 school year are now CLOSED! Applications for the 2027-2028 school year will open in October 2026 and close December 15, 2026.
Aggies Elevated uses a shared application process for both campuses. Interested students should complete ONE application whether applying for one or both campuses. You will indicate which campus(es) you are applying for on the Application Information Form and by payment of the application fee. Each campus can accept 5-7 students. Because spots are limited, we strongly recommend considering and applying to BOTH campuses!
There are several parts to the application process– each piece must be completed to be considered for admission. The Application Checklist will guide you through the steps. Incomplete applications will not be considered.
Application Steps
1. Application Fee
The application fee indicates which campus(es) you are applying to. The fee is $25 per campus. It is non-refundable and must be paid before your application will be considered complete.
2. Application Information Form
This form asks for general student and parent information, such as contact information, past educational experiences, and basic disability disclosure.

3. Student Questionnaire
These questions should be answered by the student. A parent or support person may scribe (type), but all answers must be from the student’s perspective.

4. Parent Questionnaire
One or both parents may answer these questions. Please make sure your answers are honest and representative of your perspectives and the student’s support needs.

5. Two Teacher/Service Provider Questionnaires
Share the link below with 2 teachers or service providers who are currently working (or have most recently worked) with the student and knows them well. These people must be able to speak to the student’s strengths, challenges, and support needs. Examples: special education teacher, mental health or behavioral practitioner, disability service or support provider.

6. Disability Documentation
This documentation is necessary to determine eligibility for the program and provide appropriate supports. Includes 3-year/psychoeducational evaluation, IEP, transition plan & assessments, special education summary of performance. See form for specific requirements.

7. Guardianship Documentation (If applicable)
If someone has guardianship or other legal decision-making power for the student, please provide that documentation. Students will be regarded as their own legal guardians unless documentation outlining guardianship and/or other legal decision-making authority is provided.
**Students who are their own legal guardians do NOT need to provide this documentation.

8. Academic Transcripts
From high school as well as any college, tech school, or post high programs attended.
Have school(s) send transcripts directly to aggieselevated@usu.edu or mail to Aggies Elevated, 2865 Old Main Hill, Logan UT 84322, regardless of the campus you are applying for.
9. Sign Acknowledgements & Release of Information Documents
These documents include information about the Aggies Elevated program fee, expectations, supports, and policies that applicants and parents must understand and agree to and a request for permission for Aggies Elevated to communicate with relevant sources (e.g., school districts & personnel, mental health providers, Vocational Rehabilitation, USU offices, etc.) to determine eligibility for the program.

Aggies Elevated is an Inclusive Postsecondary Education (IPSE) program designed specifically for students with Intellectual Disability (IQ of 75 or less). If a student’s primary diagnosis is not intellectual disability (for example, autism, TBI, specific learning disability, etc.), they may still be eligible if they have co-occurring intellectual disability or documentation that shows they function similarly to someone with an intellectual disability.
