Dementia Caregiver Clinic

The Dementia Caregiver Clinic is composed of an interdisciplinary team of professionals who evaluate the needs of individuals with dementia (or who are suspected of having dementia) and their caregivers. The team provides resources and referrals for additional services or supports tailored to individuals’ needs.

The interdisciplinary team of professional includes representatives from the following areas in the Sorenson Center:

Fee Information

There is no cost for the initial interdisciplinary care consultation. SCCE services received following the initial consultation are billed to the client or to insurance.

What does the process of receiving services look like in your clinic?

Once you contact us to express interest in our services, we will schedule a brief phone intake for you with our clinic coordinator or another member of our team. The purpose of this phone intake is to allow us to gather information on you / your loved one’s background and needs. This will allow us to be prepared for our initial meeting and tailor that meeting to your needs.

Following the phone intake, and depending on your needs and preferences we will schedule either: 1) a consultation meeting with one to two members of our team to discuss resources and planning; or 2) a meeting with our full interdisciplinary team for you and your person for whom you are caring. At a meeting with our full team we may perform some screenings (e.g., hearing screening, cognitive screening, mental health screening) and our team members will likely have some follow-up questions for you / your loved one. The majority of this meeting will be spent providing you with information and resources for referrals to best support you and the person for whom you are caring.

Following the consultation visit or team meeting, we will provide you and your loved one with reports summarizing our meeting and our recommendations within two weeks of the meeting. If you have follow-up questions, you are welcome to reach out to our clinic coordinator.

Does your clinic diagnose people with dementia?

No, our clinic is focused on supporting individuals with dementia (or who are suspected of having dementia) and their caregivers. Our clinic staff can conduct a very brief cognitive screening, but we do not conduct comprehensive evaluations for dementia and we cannot provide a diagnosis.

What information do I need to share ahead of time or bring to my appointment?

If your loved one has had evaluations for dementia, these could be helpful to share for the team to better understand the strengths and challenges of the person for whom you are caring.

Who qualifies as a caregiver?

A caregiver is anyone who is providing regular support for a loved one with dementia / suspected of having dementia. Caregivers may be spouses, children, siblings, friends, etc. We also welcome multiple caregivers per family when the caregiving support is shared. We understand that caregiving can be a stressful experience and we are here to support you.

How is meeting with an interdisciplinary team helpful?

Individuals with dementia often have an array of challenges and sometimes it is not clear from initial descriptions what services might be best for an individual and their caregiver. By having professionals from different disciplines, we are better able to address needs and target recommendations. We will work to tailor the make-up of our interdisciplinary team to best address the concerns you initially report. We do recognize that not everyone needs or wants to meet with a full interdisciplinary team and in those situations, we can schedule you for our consultation visits.
