Throughout your time as student there may be times that you will need to complete one or more of the following forms.We have tried to identify the most commonly used forms for students but this is not an all inclusive list. You will want to stay in contact with your advisors and faculty mentors to ensure that needed forms are completed.
- auditing a class
- credit limit increase (registering for more than 18 credits)
- changing a course to pass/fail
- regisrtation error override (i.e.: prerequisite, locations, capacity, instructor's permission, etc.)
- time conflicts.
Independent research is an alternate to classroom instruction allowing students to pursue personal research interests and gain research experience by formalizing an independent project under the guidance of a professor. It is important for an instructor and student to meet establish expectations of the requirements for this project.

Steps to complete:
- Meet with the student to discuss expectations and requirements.
- Review sections of independent study currently offered by the department in Banner.
- Determine if a new section with a unique name needs to be created.
- The faculty member is responsible to contact the main office to set up the new course.
- Complete Independent Research Form.
- Copy of form is maintained by the student, instructor, and department.
- If a student is registered for the general section of this course, the faculty will need to let the listed instructor know when the student has met the requirements.
