Licensure Only
Admission to the licensure only program requires students be a licensed level II teacher and hold a Master's degree in an education or closely related field. Applicants with Master's degrees in other areas may be required to take additional courses.

Please submit:
1__ ASC Application form (online submission)
2__Three Letters of Recommendation (Required Form) which attest to your potential as a school administrator
Mail the following application materials to: ASC Program 2800 Old Main Hill Logan UT 84322
3__$50 non-refundable application fee
4__Photograph
5__Official Graduate Transcripts which list your awarded Master's degree (We do not need these if your Master's degree is from USU)
6__Copy of current teaching license ( Level 2 license in Utah required. Must have 2 years of certified experience before being admitted to the program.)
Interview: After completing the admissions checklist, you will be invited to participate in a telephone interview with the admissions committee. The interview should take approximately 15 minutes. Rating Sheet
University Admissions: After formal acceptance to the ASC program, you will need to apply for admissions to the University through USU Undergraduate (Re)Admissions application** Note: This must be done through the Undergraduate admission office, not the grad school. **

2__ASC Application form (online submission)
